Memorial Choice

Memorial Choice provides convenient access to primary care, urgent care, prompt care, laboratory services and wellness resources through participating Memorial Health locations. Employees and covered dependents enrolled in a City medical plan may receive designated Memorial Choice services at no cost when they use an included location and the service is on the approved Memorial Choice service list.

Important: Memorial Choice is not the City's occupational health or workers' compensation provider.


Important Billing Information

Memorial Choice does not mean that every service received at a Memorial Health facility is free.

Before receiving care, confirm that:

  • The location participates in Memorial Choice.
  • The service is included on the approved service list.
  • Any laboratory testing or additional procedures are included.

Services that are not included will be processed under the regular Aetna medical plan. Employees should contact Aetna with questions about medical plan coverage or a claim.

Memorial Choice Is Not Occupational Health

Memorial Choice is a health plan benefit and should not be used for work-related injuries, workers’ compensation treatment, employment physicals or required drug and alcohol testing.

Employees needing occupational health or workers’ compensation services must follow City procedures and use the City’s designated occupational health provider. Using Memorial Health for these services may result in the employee receiving a bill.

Using Memorial Choice

When scheduling or receiving care:

  • Confirm that the provider and location participate in Memorial Choice.
  • Tell the provider you are covered by the City of Springfield health plan.
  • Present your Aetna medical identification card.
  • Ask whether the service is included under Memorial Choice before receiving care.

Services outside the approved Memorial Choice service list will be processed through the Aetna medical plan and may be subject to the applicable deductible, copay or coinsurance.

Available Services

Memorial Choice includes designated services in the following areas:

  • Primary care
  • Urgent and prompt care
  • Laboratory testing
  • Annual well-woman visits provided through a participating primary care provider
  • Wellness biometric screenings

Additional tests, procedures, specialists or services ordered during a visit may be billed separately through Aetna.

Wellness Program

To save on your health insurance premiums you are required to complete a Wellness Exam with Memorial Choice or your Primary Care Physician. To qualify for the wellness premium discount, complete both of the following by December 31:

Step 1: Complete the Health Risk Assessment

  • Download the My Memorial App from the app store (or update it to the latest version to access the HRA).
  • Build your profile and select City of Springfield as your current employer.
  • Open the menu (top left three lines) and click Check My Health.
  • Select your name under “Who is this for?” and choose Start Your Assessment.
  • Complete the “Where I Am Now” and “Where I Want to Be” sections and submit.

That’s it! If you’d like additional support, you can request a call from a nurse care coordinator.

Step 2: Complete your Biometric Screening

Option 1: Work with your Primary Care Provider

  • Print the biometric screening form and schedule an appointment.
  • Complete labs at the lab of your choice, Memorial laboratory options below.
  • Have Primary Care provider submit the completed biometric screening form to MemorialChoice@mhsil.com or fax to (217) 527-3435.

Option 2: Work with Memorial Wellness Center

  • Schedule an appointment with Memorial Wellness Center
  • Complete labs at any Memorial Laboratory location
  • Your biometric exam results will be sent to you and submitted to MemorialChoice@mhsil.com or faxed to (217) 527-3435.
